Application for Duplicate or Lost Birth Certificate

To obtain a duplicate birth certificate following loss or mutilation of the original document

 

Requirements are as follows: -

1. Application form duly completed (Download application form);
2. General search form must be duly completed (Download search form);
3. Photocopy of lost/ mutilated birth certificate;
4. Copy of applicant’s passport;
5. Proof of legal stay of parents in country of residence;
6. Application letter clearly indicating IN FULL, your residential address, email, telephone number, and postal address;
7. Processing fee of SEK 150; and
8. Fee to be paid (Click here).
